Before you leave hospital you will be given a Discharge letter about your stay and any treatment needed.
Please deliver this letter to the clinic as soon as possible so that we can arrange any care needed or adjust your medication.
An electronic copy of your discharge details can be sent by the hospital but this may take a few days to arrive.
